What is Fistula-in-Ano?

An anal fistula, which can also be referred to as a fistula-in-ano, is a passage or tract that occurs between the internal part of the anal canal and the skin near the anus. The most common cause of an anal fistula is an anal abscess, which when left unattended or incorrectly heals, results in a persisting tract through which infections can spread.

Anal Fistula Classification
The knowledge of how complicated the fistula tract is should be considered when planning treatment. Fistulas are differentiated according to how the tract passes through the muscles of the anal sphincter:

  • Intersphincteric Fistula: The most common form of fistula, which involves a tract running between the internal and external sphincter muscles.
  • Transsphincteric Fistula: A tract running straight through the sphincter muscles.
  • Suprasphincteric Fistula: A complicated tract passing above the sphincter muscles.
  • Extrasphincteric Fistula: A rare and complicated form of a fistula tract that starts in the rectum and passes through the body tissues to reach the skin.

Simple vs. Complex Fistulas
There is another classification scheme, which separates Simple Fistulas (simple, single tracts with few branches) from Complex Fistulas (multiple tracts, recurrent fistulas, or those related to systemic diseases like Crohn’s Disease and Diabetes).

Signs for Recognizing Anal Fistulas

If you experience any of the following symptoms, you should consult a doctor immediately:

  • Persistent Discharge of Pus: Frequent discharge of pus or blood.
  • Painful and Swollen: Severe pain and swelling in the region surrounding the anal canal.
  • Skin Irritation: Persistent irritation and redness due to the discharge.
  • Fevers: Usually indicate an infection or developing abscess.
  • Openings in the Skin: An open wound near the anal canal, which may intermittently close and open again.

Root Causes
Although a previous anal abscess is considered the most common cause, there are other possible contributing causes for a fistula:

  • Persistent infection within the anal glands.
  • Direct physical injury or trauma to the anorectal area.
  • Inflammatory Bowel Disease (IBD), including Crohn’s disease.
  • General medical disorders such as Tuberculosis or Diabetes, affecting immunity.

The Ayurvedic Viewpoint on Fistula-In-ano

Fistula-in-ano in Ayurveda is known as Bhagandara and is considered one of the difficult conditions to manage (Ashta Mahagada). The term Bhagandara is made up of two terms “Bhaga” (pertaining to the perineum), and “Darana,” meaning tear apart or split.

Classification of Fistula-in-ano
Fistula-in-ano can be classified into five categories according to the Doshas imbalance:

  • Vataja Bhagandara: Condition that results from intense sharp pain, dryness, and blackish appearance.
  • Pittaja Bhagandara: Pain with a burning sensation, redness, and yellowish foul discharge.
  • Kaphaja Bhagandara: Associated with whitish thick discharge, itching, and swelling.
  • Sannipataja Bhagandara: Most complicated due to the imbalanced nature of all the three Doshas.
  • Agantuka Bhagandara: Fistula not related to Dosha imbalance but caused by trauma or injury externally.
