Internal Piles: They are found deep within the rectum and cannot be seen or felt but usually result in painless bleeding.

External Piles: These develop underneath the skin around the anus.

The Four Grades of Internal Piles Explained

Internal piles can be categorized depending on how serious they are. Knowing which Grade you have is very important when determining the best way to treat it:

  • Grade 1: First Signs of Piles
    These are small lumps found within the anal canal.
  • Signs: Fresh red blood when wiping; does not cause any pain.
  • Visibility: Not seen from the outside.
  • Grade 2: Prolapsed Piles
    These piles are bigger and might protrude (prolapse) upon defecation.
  • Signs: Discharge of mucus and sensation that one has not fully relieved himself.
  • Visibility: These prolapse during defecation but automatically return into the rectum.
  • Grade 3: Manual Prolapsed Piles
    These are more advanced piles and protrude outside the anus.
  • Signs: Continuous bleeding, itching, and irritation.
  • Visibility: This Grade requires the patient to manually tuck the piles back into the rectum after a bowel movement.
  • Grade 4: Chronic Prolapsed Piles
    This Grade represents the most serious form of piles. They remain outside the anus permanently.

Signs: Excessive bleeding, difficulty sitting and moving around, and the risk of developing a thrombosis (a painful clot).

Visibility: They cannot be tucked back into the rectum.

What Leads to Hemorrhoids?

Hemorrhoids may be considered a “lifestyle disorder” because of the continuous strain put on the veins of the rectum. Some causes of hemorrhoids are:

  • Digestive Conditions: Chronic constipation and diarrhea.
  • Unhealthy Eating: Inadequate fiber intake, along with eating too much junk food or spicy foods.
  • Behavioral Patterns: Straining while using the bathroom, long hours spent sitting down, and heavy lifting.
  • Physiological Causes: Obesity, aging, and pregnancy.

Arsha Roga in Ayurveda

The term Arsha, in the ancient books of Ayurveda, denotes the disease Piles, which “attacks a man like an enemy.” It is primarily considered a digestive problem caused due to a disturbance in the Dosha system, specifically Apana Vata (which helps in the elimination process).

The intake of improper food (hot, oily, or rough food) causes a reduction in our body’s Agni, leading to the formation of Ama, resulting in constipation. This increases the pressure on the rectum veins, causing their swelling and the formation of Arsha.

Types of Arsha:

  • Vataja Arsha: Small, hard, and dry piles with sharp pain.
  • Pittaja Arsha: Soft and reddish piles with a burning sensation.
  • Kaphaja Arsha: Large, pale, moist, and very itchy piles.
